Welcome to the NextGen Transition Hub! 

What is transition?
Transition is the word used to describe the process of moving a patient from children’s (or ‘paediatric’) care to adult services. The move usually takes place between the ages of 16-18 but is different for each young person depending on their own needs and circumstances. The whole process can start as early as age 12. The aim of the transition process is to make sure that the young person and their family are fully prepared; all their care needs are properly planned for; and their new team are ready to support them.

Why focus on transition?
We’ve heard from patients, their families and their care teams, that transition can be a challenging time for everyone involved and that it’s vital to get it right to support the future health and happiness of the patient and the overall success of their treatment.
Many of you have told us that more needs to be done to improve the process and make it easier for everyone to deliver. That’s why it’s become a key focus of our NextGen project.

What are we doing?
We’re meeting with clinics across the country to understand how transition is planned and delivered and to talk to professionals about what service they’d like to provide to patients, including the challenges and barriers they might be facing.
We’re also conducting research with patients and their families to understand their experiences of transition; what’s been particularly helpful; and what they’d like to see change.
We’re using all of this different information to plan and create toolkits and resources, which can support patients and their families experiencing transition and to alleviate some of the challenges of clinics.
